Description |
A pamphlet distributed by the Unemployment Insurance Commission of Canada titled "Direction to Report and Information to Claimants" owned by Isabel Smith. On the interior of the cover is a blue stamp "M-285-739 / SMITH ISABEL / 110X729", and handwriting "hal. (house fura) 8-27 71 / Isabel Smith 11 Dec/61". The first page has most of the chart filled out in black pen, as well as a stamp with the address "72 Hurontario St., / Collingwood, Ontario." The inner back cover has a form attached by a clip. It is titled "UNEMPLOYMENT INSURANCE COMMISSION / Claimant's Weekly Report", and is stamped in red "281610". The top form is missing. The bottom form has "JUN 10 1962" stamped in black in the date field. Yellow-brown cover with white pages and black print. |
